Refund and cancellation policies

Refunds are at our discretion based on a case by case basis. CSHSMD membership is for the calendar year beginning January 1 and ending December 31. CSHSMD membership is fully refundable during the first thirty days of membership. After that point membership is not refundable.

Registration fees for CSHSMD conferences and seminars are refundable provided that a request for a refund is received 15 days prior to the event.

Refund requests must be in writing, documenting the original payment method, name of the member or conference registrant, and postmarked according to the deadlines listed above.

Annual Dues Fee (Professional) – $175.00
Annual Dues Fee (Student) – $75.00

Shipping and delivery information

Email confirmation will be sent for any dues payments or seminar registrations on the website. You will also receive a confirmation page on the website when you pay your dues or register for an event. Please print that page and save as your documentation.
